The Center for Healthcare Decisions (CHCD) is a
nonpartisan, nonprofit organization that has worked for
more than 15 years to improve care at the end of life. We
have developed materials that help people think and talk about their
end-of-life care wishes, made hundreds of presentations to community groups and
conducted many seminars for health professionals and faith leaders.
CHCD is part of the leadership team for the Sacramento Area POLST Coalition and oversees its speakers bureau. To schedule
